Important information for employers employing employees in San Francisco: the first required annual employer reporting obligation is due by April 30, 2009. Covered employers must report their health care expenditures annually using the Mandatory Annual Reporting Form.
A copy of the Mandatory Annual Reporting Form may be downloaded from the San Francisco Office of Labor Standards Enforcement’s website. This website also includes an Overview of the Reporting Form, Instructions for Annual Reporting Form, as well as the “fill-able” on-line Form (2008 Annual Reporting Form).
Covered employers who fail to return the completed form by the required date will be in violation of the law and be subject to penalties and other corrective action.
